The tiering means your cheapest months come early, during titration, when you’re still confirming whether the medication works for you. By the time you reach a maintenance dose, you’re at the $299 to $349 range. One month is defined as a bottle of 30 tablets.

What you pay with insurance or Medicare

If your commercial plan covers Foundayo, the Foundayo Savings Card can drop your out-of-pocket cost to as little as $25 a month, with a maximum savings cap and a card that runs through the end of 2026. If your commercial plan doesn’t cover it, the same card caps your cash cost at roughly the self-pay numbers above. Government beneficiaries are excluded from the savings card, so this route is for people with private, commercial coverage.

Medicare is a separate story. Foundayo is one of the three drugs included in the Medicare GLP-1 Bridge, alongside Wegovy and Zepbound, which means eligible Part D members may access it for about $50 a month for weight management beginning July 1, 2026, with a prior authorization and clinical criteria. Medicaid coverage varies by state.

Why a pill, and what you trade for it

The appeal of Foundayo is convenience. It’s a small-molecule, non-peptide GLP-1 receptor agonist, which is a different chemistry from the injectable peptides. That lets it be taken any time of day without the fasting window that the older oral option, Rybelsus, requires. It also doesn’t need refrigeration.

The honest tradeoff is efficacy. Foundayo’s approval rested on Lilly’s ATTAIN program, where the highest dose produced roughly 12% average weight loss over 72 weeks, about 27 pounds. A few practical notes. Foundayo carries a boxed warning for thyroid C-cell tumors and isn’t for anyone with a personal or family history of medullary thyroid cancer or MEN-2. The most common side effects are gastrointestinal: nausea, constipation, diarrhea, and vomiting, plus headache and some reports of hair loss. A word on “compounded Foundayo”

You can’t get a legitimate compounded version of orforglipron. Compounded semaglutide and tirzepatide exist because those drugs spent time on the FDA shortage list, which opened a temporary pathway. Orforglipron is a different molecule that has never been in shortage, so any site claiming to sell compounded orforglipron in 2026 should be treated as suspect.
