Tirzepatide produced 20.2% average weight loss versus 13.7% for semaglutide, roughly 47% more weight lost. Tirzepatide users were also more likely to hit milestones like 20% and 25% weight loss. Interestingly, gastrointestinal side effects leading to discontinuation were somewhat less common with tirzepatide (2.7%) than semaglutide (5.6%). So on both efficacy and this measure of tolerability, Zepbound had the advantage in this trial.

Where Wegovy Has the Edge

Weight loss isn’t the whole story. Wegovy has a broader set of FDA-approved uses: beyond obesity, it’s approved to reduce cardiovascular risk (heart attack and stroke) in people with established heart disease, and more recently for MASH, a serious fatty liver disease. Zepbound, meanwhile, is approved for obstructive sleep apnea in addition to obesity. So the “better” drug partly depends on your health picture. Consider a hypothetical patient with obesity and established heart disease: Wegovy’s cardiovascular indication might make it the more fitting choice, even though Zepbound produces more weight loss on average.
