My Tirzepatide Vial Looks Cloudy. Is It Still Good?
If your tirzepatide vial looks cloudy, do not inject it. Tirzepatide solution should be clear and colorless to slightly yellow, with no visible particles. Cloudiness, haze, floating specks, or a change in color are all signs that something has happened to the product, and none of them are things you can correct at home. Set the vial aside, do not shake it in an attempt to clear it, and contact the pharmacy that dispensed it for a replacement.

What cloudy actually means in a peptide solution

It got too warm
The most common story is a break in the cold chain. A shipment sat on a porch through a hot afternoon. A cooler pack melted earlier than expected. The vial rode in a car for a few hours in July. Heat is the workhorse cause of aggregation in peptide products.
It froze
Freezing is worse than most people assume, and it happens more often than people expect, because household refrigerators are not as stable as they look. A vial pushed to the back of a shelf near the cooling vent can dip below freezing overnight without the rest of the fridge showing any sign. Ice crystal formation is mechanically destructive to peptides, and a product that has frozen and thawed will sometimes look fine at first glance and turn hazy later.

It was shaken
Vigorous shaking creates air and liquid interfaces that peptides are drawn to and unfold at. Rolling a vial gently between your palms is fine. Shaking it like a cocktail is not.
It is simply old
Every compounded preparation carries a beyond-use date, and that date reflects how long the pharmacy has data to support the product remaining stable. Past it, appearance changes are more likely.
What a normal vial should look like
Hold it against a white background in good light and look for:
- Clarity. You should be able to read text through the liquid.
- Color. Clear and colorless through very slightly yellow is expected. Brown, pink, or noticeably darker yellow is not.
- No particles. No floaters, no flakes, no strands, no sediment at the bottom.
- An intact stopper and seal. No cracks in the glass, no dislodged crimp, no dampness around the neck.
Consider this scenario: a patient pulls a vial out on injection day and notices the liquid looks faintly milky in the kitchen light, then decides it looks fine when held near a window. That inconsistency is itself the answer. A clear solution looks clear in every light. If you find yourself hunting for an angle where it looks acceptable, treat it as cloudy.
What not to do
Do not shake it to redissolve the haze. Aggregation is not a settling problem, and shaking makes it worse.
Do not warm it up to clear it. Heat is one of the causes, not a remedy.
Do not inject “just this once” while you wait for a replacement. A skipped week is a manageable inconvenience. Injecting a compromised product is not a trade worth making.
Do not filter it. Nothing you can do at home makes an unstable protein solution safe again.
What to do instead
- Photograph the vial against a plain background, including the label with the lot number and beyond-use date. This makes the pharmacy conversation faster.
- Note anything you know about how it was stored, including shipment timing, whether the cooler was still cold on arrival, and where in the fridge it lived.
- Call the dispensing pharmacy. Reputable compounders replace product that arrives or turns out compromised, and they will want the lot information.
- Ask your provider whether to adjust your injection day for the week, rather than deciding on your own.

Does this happen more with compounded tirzepatide than with the brand?
Not inherently. The molecule behaves the same way regardless of who prepared it. What differs is the supply chain and the format. Compounded products more often reach patients as multi-dose vials rather than sealed single-use pens, which means more handling, more time at room temperature during each use, and more opportunities for a storage mistake.
