The Mid-Autumn Festival on Mounjaro
The Mid-Autumn Festival falls on the 15th day of the eighth lunar month, usually in September or early October, when the moon is at its fullest. Chinese, Vietnamese, Malaysian, Singaporean, and many other families celebrate with a reunion dinner, moon viewing, lanterns, and, above all, mooncakes. Boxes of them arrive from friends, relatives, and coworkers, filled with lotus seed paste, red bean, mixed nuts, or salted egg yolks. Pomelo, taro, persimmons, and pots of tea fill out the table. In Vietnam, Tết Trung Thu is also a festival for children, with lantern parades and lion dances. Customs vary by family and region.
If you take Mounjaro or compounded tirzepatide, a rich reunion dinner followed by dense mooncakes on a smaller appetite can leave you uncomfortable. If you have type 2 diabetes, all that sugar matters too. Here is how to enjoy the festival.
Plan your dose around the festival
In SURMOUNT-1, a 72-week trial of tirzepatide, the same active ingredient as Mounjaro, in adults with obesity or overweight who did not have diabetes, the most common adverse events with tirzepatide were gastrointestinal, and most were mild to moderate in severity, occurring primarily during dose escalation.
That makes the timing of increases the most useful thing to plan. If a dose increase would fall in the week of the festival, ask your provider whether to hold it until afterward. Our guide on when to increase your tirzepatide dose covers how those decisions are usually made. The Mounjaro label allows the day of weekly administration to be changed as long as the two doses are at least 3 days (72 hours) apart, but ask your provider before you change it.
Mooncakes: share the wedge
A traditional mooncake is small but very rich, with a dense, sweet filling and often a salted egg yolk in the center. Families usually cut them into wedges to share, and that tradition suits Mounjaro well. Enjoy one thin wedge with a cup of tea, ideally after your meal rather than on its own. If you have type 2 diabetes, eating sweets after food rather than on an empty stomach is easier on your blood sugar. Snow skin mooncakes are lighter in texture, but they are still sweet, so the same approach applies.
The reunion dinner
Many reunion dinners include dishes that work well on a small appetite. Steamed fish, tofu, chicken, shrimp, and soups bring protein, so start with those. Keep rice, noodles, and rich braised or fried dishes to small portions, eat slowly, and stop at the first sign of fullness.
Fruit and tea
Pomelo, persimmons, and other seasonal fruit are refreshing after a rich meal and lighter than another mooncake. Tea is a natural partner for mooncakes and a gentle choice if your stomach feels unsettled.
All those gift boxes
You do not have to eat every mooncake you are given. Share them with neighbors, bring them to gatherings, or pass them along to friends. Many families already circulate mooncake boxes, so regifting is part of the tradition.
Lantern walks and evening blood sugar
The prescribing information for Mounjaro says people who take it with insulin or an insulin secretagogue, such as a sulfonylurea, may have an increased risk of hypoglycemia, including severe hypoglycemia. A late dinner and an evening walk with lanterns can change your usual pattern. If that is you, keep glucose tablets in your pocket and know the signs: shakiness, sweating, a fast heartbeat, sudden hunger, irritability, or confusion. Never change those medications on your own.
If you feel queasy
In the placebo-controlled trials for Mounjaro in adults with type 2 diabetes, nausea was reported by 12% to 18% of adults across the three doses, compared with 4% on placebo. A rich dinner followed by mooncake is a common trigger. If nausea hits, step away from the table, sip warm tea or water, and rest. If you switched from semaglutide, our guide comparing tirzepatide and semaglutide side effects covers how they differ.
Protein through the season
Mooncake season can stretch over weeks of gatherings, and it is easy to fill a small appetite with sweets instead of meals. Eat protein at every meal, and keep some strength training in your week. Some of the weight lost on these medications is muscle, and protein helps protect it. Our guide on getting the most out of your GLP-1 treatment covers why that matters.
When relatives notice
Reunion dinners bring together relatives who may comment on your weight or your small plate. You do not owe anyone an explanation. “Everything is delicious, I’m just full” is enough. If you want to say more, “I’m on a medication that makes me full quickly” is short and factual. Offering to serve tea or cut the mooncakes for everyone keeps your hands busy and your plate light.
Track how you feel
Keep a simple log during the festival of what you ate, how you felt, and any blood sugar readings, and share it with your clinician. Watch for the signs of under-eating too: persistent fatigue, feeling cold, hair shedding, dizziness when you stand, losing strength, and losing weight faster than you and your provider intended. Our guide on tracking your progress covers ways to record more than your weight.
Consider this scenario: a woman on Mounjaro with type 2 diabetes steps up her dose four days before the Mid-Autumn Festival. At the reunion dinner she eats a full bowl of rice and several rich dishes, then two whole lotus seed mooncakes with egg yolk because relatives keep offering them. That night she is nauseated and her blood sugar is high. Holding the dose increase, starting with steamed fish and tofu, sharing a thin wedge of mooncake with tea, and passing the extra boxes to neighbors would very likely have made it a happier night.
The short version
For the Mid-Autumn Festival on Mounjaro, ask your provider about holding a dose increase, start the reunion dinner with fish, tofu, or chicken, and keep rice and rich dishes small. Enjoy mooncake one thin wedge at a time with tea after your meal, share or regift extra boxes, and carry glucose tablets if you take insulin or a sulfonylurea.
