Wegovy Rash: When to Worry

Introduction

Rash on Wegovy® is uncommon but documented. In STEP 1 (Wilding et al. 2021 NEJM), injection site reactions were reported by about 3 to 4 percent of semaglutide 2.4 mg users versus about 1 to 2 percent on placebo. Most were mild, local, and resolved within a few days. True systemic hypersensitivity reactions are much rarer and need urgent care.

The most common Wegovy rash is a red, itchy patch at the injection site. It is not an allergy, it is local irritation. Generalized rash, hives, or rash with swelling is a different category entirely. That is when you stop the next dose and call your prescriber, or go to the ER if breathing is affected.

At TrimRx, we believe that understanding your options is the first step toward a more manageable health journey. You can take the free assessment quiz if you’re ready to see whether a personalized program is a fit for you.

What Does a Wegovy Rash Look Like?

Most commonly, a red or pink patch at the injection site, 1 to 3 inches across. Can be itchy or warm. Sometimes slightly raised. The rash appears within hours of injection and fades over 1 to 5 days.

Quick Answer: Injection site reactions affect 3 to 4 percent of Wegovy users in STEP 1

A small bruise around the rash is common and not concerning. Mild swelling is also normal. A small lump under the skin (a nodule) can persist for a week or two, especially if you inject in the same spot repeatedly.

Rash that spreads beyond the injection site, covers multiple body areas, or comes with hives elsewhere is different. That is a systemic response and needs attention.

Is Wegovy Rash an Allergic Reaction?

Usually not. Most injection site rashes are local irritation. The skin reacts to the needle, the volume injected, or the excipients. That is different from an IgE-mediated allergy to semaglutide.

True hypersensitivity reactions to semaglutide are listed in the Wegovy label as rare but possible. They can include generalized rash, hives, swelling of the face or throat, and rarely anaphylaxis. These are urgent.

If you have had a serious allergic reaction to semaglutide before, Wegovy is contraindicated.

How Common Is Wegovy Rash?

In STEP 1, injection site reactions were 3 to 4 percent at the 2.4 mg dose, versus 1 to 2 percent on placebo. In SELECT (Lincoff et al. 2023 NEJM), with 17,604 participants over 39.8 months, injection site reactions were also in the low single digits and severe reactions remained rare.

Generalized rash is much less common, well under 1 percent based on GLP-1 trial data. For comparison, nausea hit 44 percent and diarrhea 31 percent in STEP 1. Rash is a real but small signal.

When Does Wegovy Rash Usually Appear?

Local rashes appear within hours of injection. They peak by 24 hours and fade by 3 to 5 days. Some people see it with the first dose, others not until later doses.

Generalized rash, if it occurs, almost always shows up in the first 1 to 4 doses. A new generalized rash after months of stable Wegovy use is less likely to be drug-related and more likely to need a workup for other causes (viral, contact dermatitis, food allergen).

What Causes Wegovy Injection Site Rash?

Four common drivers. Repeated injections in the same skin patch without rotation. Injecting too shallow (into the dermis instead of subcutaneous fat). Injecting cold pen straight from the fridge without warming. Wiping with alcohol and not letting it dry before injecting.

Switching arms, thighs, and abdomen weekly fixes most cases. Letting the pen sit at room temperature for 30 minutes reduces sting and rash. Letting alcohol fully dry (about 30 seconds) before injecting helps too.

How Do You Treat a Wegovy Injection Site Rash?

Most need nothing. Cool compress for 10 minutes if it itches. Hydrocortisone 1 percent cream for 2 to 3 days for persistent itching. Oral antihistamine (cetirizine 10 mg or diphenhydramine 25 mg) if widespread or very itchy.

Do not pick or scratch. If the rash is hot, expanding, oozing pus, or has red streaks running away from the site, that is a possible infection and needs medical evaluation.

Key Takeaway: Generalized rash, hives, or swelling needs urgent medical evaluation

When Is a Wegovy Rash an Emergency?

When it comes with any of these: swelling of the face, lips, tongue, or throat. Trouble breathing or swallowing. Wheezing or chest tightness. Hives spreading rapidly across the body. Dizziness, fainting, or rapid heart rate alongside the rash.

Any of these is a possible anaphylactic reaction. Call 911 or go to the nearest emergency department. Do not wait. Anaphylaxis is rare with GLP-1 drugs but is in the prescribing information.

Can You Continue Wegovy After a Rash?

For local injection site reactions, yes, almost always. Rotate sites, warm the pen, let alcohol dry. The rash rate usually drops within 2 to 3 injections.

For generalized rash or hypersensitivity, no. Do not restart without prescriber evaluation. In some cases, switching to a different GLP-1 like tirzepatide is an option since the molecules differ.

How to Prevent Wegovy Rash

Five practical steps. Rotate sites weekly: abdomen one week, thigh next, arm next. Let the pen sit out for 30 minutes before injecting. Clean the site with alcohol and let it dry fully before piercing the skin.

Inject at 90 degrees into a pinched fold of skin, deep enough to reach subcutaneous fat. Hold the injection for the full count the pen requires. Do not rub the site after injecting.

If you still get rashes despite all this, talk to your prescriber. A TrimRx clinician can review your technique in a personalized treatment plan.

When Should You See a Doctor About Wegovy Rash?

Seven signals. Rash covering more than the immediate injection site. Hives or itching elsewhere on the body. Swelling of the face, lips, tongue, or throat. Trouble breathing or swallowing.

Red streaks running away from the injection site. Rash with fever. Rash lasting more than 7 days without fading. Repeat rash at the same site despite rotation. A free assessment quiz can help identify whether a switch in approach makes sense.

Bottom line: Severe allergic reactions are listed in the Wegovy label as rare but possible

FAQ

Is Wegovy Rash Dangerous?

Usually not. Local rashes are mild. Generalized rash or rash with swelling is urgent.

Can I Take Benadryl for Wegovy Rash?

Yes, for itching. Diphenhydramine 25 to 50 mg or cetirizine 10 mg helps. Stop and call a doctor if rash spreads.

Does Wegovy Rash Mean I Am Allergic?

Usually not. Most are local irritation, not allergy.

Can I Keep Injecting in the Same Arm If It Has a Rash?

No. Rotate to a different site until the rash resolves.

Why Do I Only Get a Rash Some Weeks?

Usually technique: too shallow, pen too cold, alcohol not dry, or repeat site use.

Should I Stop Wegovy If I Get a Rash?

For local rash, no. For generalized rash or rash with any swelling or breathing trouble, yes, and call your prescriber or 911 immediately.
