CagriSema vs Wegovy: How They Compare
CagriSema and Wegovy share an active ingredient, which makes this an unusually direct comparison. CagriSema is Wegovy’s semaglutide plus a second drug, the amylin analog cagrilintide. In a head-to-head trial CagriSema produced more weight loss than Wegovy (about 22.7% versus 16%), but Wegovy is FDA approved and available today while CagriSema is still investigational and cannot be prescribed. They share an ingredient
This comparison is unusual because the two drugs overlap. Wegovy is semaglutide 2.4 mg. CagriSema is that same semaglutide 2.4 mg plus cagrilintide 2.4 mg (an amylin analog) in one injection. So CagriSema is, in effect, Wegovy with a second appetite mechanism added on.
Because of that overlap, the REDEFINE 1 trial offered a clean comparison: it tested CagriSema against semaglutide alone in the same study. CagriSema produced about 22.7% average weight loss, while the semaglutide arm landed around 16%, in line with Wegovy’s own STEP-1 data. Those extra five to six percentage points are what the added amylin component contributes.

Wegovy’s edge: a proven track record
More weight loss is not the only thing that matters. Wegovy carries an FDA-approved cardiovascular benefit, meaning it is proven to reduce the risk of heart attack and stroke in certain people, based on the SELECT trial. CagriSema has a cardiovascular outcomes trial still underway, so that benefit is not yet established for the combination. Wegovy also has years of real-world use behind it and a well-documented results timeline, while CagriSema has only been studied in trials.
Availability and cost
Wegovy can be prescribed now. CagriSema cannot, and is not expected to reach pharmacies before late 2026 or 2027 even if approved. Cost is also unknown for CagriSema, while Wegovy’s pricing, including cash-pay options, is at least a known quantity today. Any “CagriSema” sold online now is not the real, regulated product.
Side effects
The two have similar gastrointestinal side effects, which makes sense given the shared semaglutide base: nausea, constipation, and vomiting, mostly during dose escalation. CagriSema’s rates were broadly comparable to what is seen with semaglutide, with low discontinuation due to side effects.
Which is the better choice?
Consider a scenario where someone is choosing between starting Wegovy now and waiting for CagriSema’s extra few points of weight loss. The added efficacy is real, but it comes with a wait of a year or more for an unapproved drug, and you would be giving up Wegovy’s proven heart benefit and established record in the meantime.
