Heat Wave on Zepbound: Staying Safe When It Gets Hot

Heat waves are hard on everyone, but they are particularly hard on people who are already running low on fluids. On Zepbound or compounded tirzepatide, many people are in exactly that position without realizing it. They eat less, so they get less water from food. Some find they simply do not feel thirsty. And the most common side effects of the medication, nausea, vomiting, and diarrhea, all cost fluid.

Put that together with several days of extreme heat, and dehydration can build quickly. This guide covers why it matters and what to do.

Why dehydration matters on Zepbound

The Zepbound prescribing information includes a warning about acute kidney injury due to volume depletion, which in plain terms means kidney problems caused by losing too much fluid. It tells clinicians to monitor kidney function in patients reporting side effects that could lead to dehydration.

The side effects in question are common. In the weight-reduction trials described in the Zepbound label, diarrhea was reported by 19% to 23% of people on the medication, depending on dose, against 8% on placebo, and vomiting by 8% to 13% against 2%.

On an ordinary day, mild diarrhea is an inconvenience. During a heat wave, when you are already sweating more than usual, it can tip you into dehydration much faster.

Drink on a schedule

On these medications, thirst is not a reliable guide.

Keep water with you all day, and drink regularly whether you feel thirsty or not.

Sip rather than gulp. Large amounts at once can be uncomfortable on a slowed stomach.

Use oral rehydration salts if you have had diarrhea or vomiting, or have been sweating heavily.

Eat water-rich foods, such as fruit, vegetables, soups, and yogurt, which also help when your appetite is small.

Go easy on alcohol, which adds to fluid loss. Your old tolerance is not a reliable guide on these medications either.

Watch caffeine if it is replacing water rather than adding to it.

Know the warning signs

Signs that you are getting dehydrated or overheated include:

  • Dark urine, or passing very little urine
  • Headache or dizziness, especially when standing
  • Muscle cramps
  • Feeling unusually tired, weak, or confused
  • Nausea that is getting worse

If you notice these, move somewhere cool, drink fluids, and rest. Contact your provider or seek medical help if you cannot keep fluids down, are passing very little urine, feel confused or faint, or symptoms are not improving.

When you have side effects in the heat

If you have vomiting or diarrhea during a heat wave, take it more seriously than you otherwise would.

Replace fluids actively, with water and rehydration salts.

Stay somewhere cool.

Contact your provider if you cannot keep fluids down for several hours, or diarrhea is frequent.

Ask your provider before your next dose if you have been unwell. Do not simply skip it and say nothing.

Plan your dose around the forecast

Side effects tend to be strongest after a dose increase and in the day or two after an injection.

Avoid stepping up your dose right before or during a heat wave if you can. Ask your provider whether to hold. Our guide on when to increase your tirzepatide dose covers how those decisions are usually made.

If you are just starting, the first weeks often bring the strongest digestive side effects. Starting in a cooler stretch can make sense. Our guide on the starting dose of tirzepatide covers what to expect.

Keep cool

Stay indoors in air conditioning during the hottest part of the day if you can. If you do not have it at home, libraries, malls, and cooling centers can help.

Do outdoor tasks early or late.

Wear loose, light clothing and a hat outside.

Check on others. Older relatives and neighbors, especially those on these medications, may be at higher risk.

Exercise in the heat

If you exercise, move it to early morning or evening, or indoors. Eat something before you go, carry water, and stop if you feel dizzy, lightheaded, or unusually tired. Dizziness is among the reported side effects of Zepbound, and heat and low fluids make it more likely.

Other medications

If you take blood pressure medications or diuretics, a heat wave combined with weight loss and lower fluid intake can make dizziness more likely. Talk to your provider, and do not change other medications on your own.

Your medication in the heat

Zepbound and compounded tirzepatide are both sensitive to heat. Do not leave pens or vials in a car, a sunny window, or anywhere that gets hot. Follow the storage instructions that came with your medication, and ask your pharmacy if you are unsure. Our guide on storing compounded semaglutide or tirzepatide at home covers the basics for compounded products.

Consider this scenario: someone on Zepbound steps up to a higher dose at the start of a week-long heat wave. They are eating little, drinking less than usual because they are not thirsty, and on day two they develop diarrhea. They keep working outside in the afternoons. By day four they feel dizzy, weak, and are passing very little urine. Holding the dose increase, drinking on a schedule, using rehydration salts, staying cool, and calling their provider when the diarrhea started would very likely have prevented it.

The short version

Drink water on a schedule, sip steadily, eat water-rich foods, and use rehydration salts after diarrhea, vomiting, or heavy sweating. Know the warning signs and act early. Hold dose increases during a heat wave, stay cool, and move exercise to cooler times. Take vomiting or diarrhea more seriously in hot weather and contact your provider if you cannot keep fluids down. Keep your medication out of the heat.
