Pharmacists on Zepbound: Managing a Relentless Shift

Pharmacists and pharmacy technicians know these medications better than most people. You counsel patients on them, field questions about side effects, and handle the prescriptions every day. But knowing a medication is not the same as taking it while working a 12-hour shift on your feet, with no real lunch break, a line at the counter, and the phone ringing nonstop.

If you take Zepbound or compounded tirzepatide, the challenges are familiar: digestive side effects at the counter, going all day without eating, and standing for hours. This guide covers how to manage them.

What the trial data tells you about timing

In SURMOUNT-1, the main trial behind Zepbound’s approval for weight management, adults with obesity or overweight took tirzepatide or placebo for 72 weeks. Average weight reductions were 15.0%, 19.5%, and 20.9% on the 5 mg, 10 mg, and 15 mg doses, against 3.1% on placebo.

The most common adverse events with tirzepatide were gastrointestinal, and most were mild to moderate in severity, occurring primarily during dose escalation.

That last point is the useful one for planning your work life. The weeks after starting and after each step up are when side effects are most likely, which means you can often schedule the hardest stretch around your rotation.

Plan dose increases around your schedule

Eat when there is no lunch break

Many community pharmacists work long shifts without a protected break. On an appetite-suppressing medication, that can mean going from 8 a.m. to 8 p.m. on coffee and a few bites.

Eat breakfast before your shift, with protein.

Keep quick protein within reach in the back: Greek yogurt, cheese sticks, nuts, jerky, protein shakes, hard-boiled eggs.

Eat a few bites between patients on a schedule rather than waiting for a break that never comes. Set a reminder if you need to.

Drink water steadily through the shift.

Go easy on coffee on an empty stomach.

Nausea at the counter

Notice your pattern. Many people find nausea follows their injection day.

Avoid heavy, greasy meals before or during a shift.

Keep plain snacks nearby, such as crackers, for queasy moments.

Step back briefly if you need to. A few slow breaths and a sip of water usually help.

Standing all day

Twelve hours on your feet, often on a hard floor, can make some people lightheaded, especially if they have eaten or drunk little. Dizziness is among the reported side effects of tirzepatide.

Wear supportive shoes and consider compression socks and an anti-fatigue mat.

Shift your weight and move when you are standing at the verification station.

Sit down briefly if you feel lightheaded, and eat or drink something.

Bathroom access

Diarrhea is a common side effect, particularly early in treatment and after dose increases. If you work alone in the pharmacy, stepping away is harder. Plan with your technician or a colleague for cover, and keep in mind that the first weeks at a new dose are when you are most likely to need it.

Workplace accommodations

Every employer has its own policies on breaks and staffing. If you need something specific because of a medical condition, such as a protected break to eat, talk to your manager or human resources about your options. You do not have to share details of your medication.

Consider this scenario: a pharmacist on Zepbound steps up to the next dose on a Monday, the start of a run of four 12-hour shifts. They skip breakfast, drink coffee all morning, and do not eat until the evening. By mid-afternoon on Tuesday they are nauseated at the counter, lightheaded, and struggling to concentrate on verification. Timing the increase before days off, eating breakfast, keeping protein within reach, eating a few bites on a schedule, and drinking water would very likely have made those shifts manageable.

When patients ask you about it
