What to Know During Recovery

A broken wrist, ankle, or foot turns ordinary life upside down for weeks. You may be in a cast or boot, on crutches, taking pain medication, and moving far less than usual. If you are on Wegovy or compounded semaglutide, a few things about your recovery need extra thought.

Whether to keep taking your medication is a question for your prescriber and the team treating the fracture, not one to settle on your own. Here is what to raise with them and what to watch for at home.

Tell everyone who is treating you

Tell the emergency department, urgent care, and your orthopedic team that you take Wegovy, at what dose, and which day you last injected. Tell your Wegovy prescriber about the fracture, too.

If the break needs surgery, this matters even more. Multi-society clinical guidance published in 2025 on using these medications around procedures moved away from blanket rules toward individual assessment, weighing things like whether you are still escalating your dose, how high the dose is, and whether you have gastrointestinal symptoms. It also covers reducing risk with measures such as a liquid diet beforehand and anesthesia planning. Tell your team what you take; they set the plan.

Do not stop or skip on your own

It can be tempting to pause treatment while everything else is chaotic. Stopping and restarting has consequences, including possibly stepping back down the dose ladder. Make that decision with your prescriber. Our guide on taking a break and restarting covers what it involves.

Pain medication and constipation

This is the problem most likely to catch you out. In the trials for Wegovy, constipation was reported by 24% of adults on the medication compared with 11% on placebo. Opioid pain medicines are well known for causing constipation as well. Add a sudden drop in movement and many people end up badly constipated within days.

Ask for a bowel plan when the pain medication is prescribed, not once you are already uncomfortable. Ask the prescriber or pharmacist what they recommend for you.

Drink water steadily through the day.

Include fiber you tolerate, such as fruit, oatmeal, and vegetables, in small amounts.

Call for advice if you have severe abdominal pain, a swollen belly, or vomiting, or if you have gone several days without a bowel movement despite your plan.

Pain medicine can also upset the stomach, and nausea is already common on Wegovy, reported by 44% of adults on the medication compared with 16% on placebo. Take pain pills with food if the instructions say to.

Eat enough to heal

Healing bone and keeping muscle while you are less active both need fuel. A smaller appetite makes that harder. Eat something small and often rather than waiting for hunger, and include protein each time: eggs, yogurt, cheese, fish, chicken, beans, or a protein drink.

Watch for the signs of under-eating: persistent fatigue, feeling cold, hair shedding, dizziness when you stand, losing strength, or losing weight faster than you and your provider intended. A fracture is not the time to let your intake slide further.

Injecting with a cast, sling, or boot

The label lists the abdomen, thigh, and upper arm as injection sites. If one arm is in a sling, the abdomen is usually the easiest site to reach one-handed. If a leg is in a cast, use the other thigh, your abdomen, or ask someone to help with your upper arm. Avoid skin that is bruised, swollen, or tender from the injury.

Our guide on GLP-1 injection sites covers how to rotate between sites.

Crutches, dizziness, and falls

In the trials, dizziness was reported by 8% of adults on Wegovy compared with 4% on placebo. On crutches or a knee scooter, a lightheaded moment can mean a second fall. Stand up slowly, sit on the edge of the bed for a moment first, eat before physical therapy, and keep water nearby.

Hold dose increases during recovery

If a dose increase is due in the first weeks after the fracture, ask your prescriber whether to hold it. Side effects tend to be strongest after each increase, and you do not need extra nausea on top of pain pills. Our guide on semaglutide dose escalation covers how the steps usually work.

Ask about your bones and muscle

The prescribing information notes that in the cardiovascular outcomes trial, patients aged 75 and older reported more hip and pelvis fractures on the medication than on placebo. That is a trial finding, not a prediction, and not every reader is 75. But if your bone broke in a fall from standing height, ask your doctor whether your bone health should be checked.

Muscle shrinks quickly when a limb is immobilized. Do the exercises your physical therapist gives you, and keep including protein at every meal.

When to call your doctor

Call your doctor or seek urgent care for calf pain or swelling, sudden shortness of breath, chest pain, severe abdominal pain, vomiting that stops you keeping fluids down, or fainting.

Consider this scenario: someone breaks their wrist on a Saturday, leaves urgent care with opioid pain pills, and does not mention Wegovy. Their scheduled dose increase falls the next Tuesday, and they take it. Eating one-handed is awkward, so they live on crackers and coffee. By Friday they have not had a bowel movement in five days and are vomiting. Telling urgent care and their prescriber about Wegovy, asking to hold the increase, starting a bowel plan with the first pain pill, and keeping easy protein and water within reach would very likely have prevented the emergency visit.

The short version

Tell every clinician treating your fracture that you take Wegovy, and ask your prescriber before stopping or changing anything. Plan ahead for constipation from pain pills, eat small amounts of protein often, inject at a site you can reach safely, stand up slowly on crutches, and hold dose increases during recovery. Know the warning signs that need a call.

If you want a provider who will adjust your plan around an injury, TrimRx includes ongoing provider access alongside treatment.

This information is for educational purposes and is not medical advice.
