For many, it is a time of prayer, fasting, and celebration. Some people eat one meal a day, some eat only fruit and milk, and others avoid grains, onion, and garlic and eat vrat foods such as sabudana, kuttu or singhara flour, potatoes, makhana, paneer, and yogurt. In many communities, evenings bring hours of garba and dandiya. Customs vary widely by family and region.

If you take Mounjaro or compounded tirzepatide, nine days of fasting on top of a smaller appetite can leave you short on food and energy. If you have type 2 diabetes and also take insulin or a sulfonylurea, fasting can bring a real risk of low blood sugar. Fasting is a personal practice, and many families adapt it for age and health. The decision about how to fast belongs to you, your clinician, and your family or priest.

Talk to your clinician before the first day

The prescribing information for Mounjaro says people who take it with insulin or an insulin secretagogue, such as a sulfonylurea, may have an increased risk of hypoglycemia, including severe hypoglycemia. Long gaps between meals make that risk higher.

If you take either kind of medicine, talk to your prescriber a week or two before Navratri begins. Tell them how you plan to fast, and ask whether anything should be adjusted and how often to check your blood sugar. Your prescriber may change your plan for the fasting days. Never change those medications on your own.

Why fasting is different on Mounjaro

On Mounjaro, you feel full after much smaller portions. During a fast, the one meal you eat may be even smaller than usual, and it may be mostly starch, such as sabudana or potatoes. Over nine days, that can add up to far too little protein.

Watch for the signs of under-eating: persistent fatigue, feeling cold, hair shedding, dizziness when you stand, losing strength, and losing weight faster than you and your provider intended. If you notice them, talk to your provider and your family about adapting the fast.

Choose a fast that fits, built around protein

Many people on these medications find a partial fast works better than a strict one, such as eating fruit and dairy through the day plus one vrat meal. Whatever form you choose, build each meal around the protein foods your tradition allows:

Paneer, yogurt, and milk

Makhana, peanuts, almonds, and other nuts

Rajgira (amaranth) and kuttu, which have more protein than most vrat flours

Seeds, where your family’s practice permits them

Eat the protein first, while your appetite lasts.

Go easy on fried vrat foods

Sabudana vada, kuttu puri, and fried potatoes are festival favorites, but heavy, fried foods are the ones most likely to sit badly on Mounjaro. Choose lighter versions when you can, such as sabudana khichdi made with less oil, roasted makhana, or paneer with vegetables, and keep fried treats to a few bites. Our guide on what to eat after an injection has ideas for foods that tend to sit well.

Garba and dandiya nights

Dancing for hours at night after a day of fasting is a lot to ask of your body. Eat your vrat meal before you go, carry water and a small snack such as nuts or fruit, and take breaks between rounds. If you take insulin or a sulfonylurea, keep glucose tablets or juice with you and tell a friend what to watch for. Signs of low blood sugar include shakiness, sweating beyond what the dancing explains, a fast heartbeat, sudden hunger, irritability, or confusion. Stand up slowly after sitting, and step out of the circle if you feel lightheaded.

When to break the fast

Break the fast and treat it right away if you have signs of low blood sugar, feel faint or confused, are vomiting, or cannot keep fluids down. Ending a fast early for your health is a reasonable choice, and one many families support. Talk to your clinician before you fast again.

Dose timing during the nine days

Side effects tend to be strongest after each dose increase. If an increase would fall during Navratri, ask your provider whether to hold it until after the festival. Our guide on when to increase your tirzepatide dose covers how those decisions are usually made. The Mounjaro label allows the day of weekly administration to be changed as long as the two doses are at least 3 days (72 hours) apart, but ask your provider before changing it.

Do not skip doses as part of the fast

Skipping your medication is not part of fasting. If you miss a dose, the Mounjaro label says to take it as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and take the next dose on your regular day. Our guide on taking a break and restarting covers why stopping and restarting is more involved than it sounds.

Festival sweets afterward

The end of Navratri brings celebrations and sweets, and in autumn, Dussehra and then Diwali follow close behind. After days of lighter eating, start with small portions of rich sweets, eat them after a meal rather than on their own, and stop at the first sign of fullness.

Keep a simple log

Write down what you eat each day, how you feel, and any blood sugar readings if you check them. Share the log with your clinician after the festival, so you can plan the next Navratri together. Our guide on tracking your progress covers ways to record more than your weight.

Consider this scenario: a woman on Mounjaro who also takes a sulfonylurea keeps a strict fruit-only fast for all nine days without talking to her doctor. With her small appetite, she eats very little fruit, and on the fourth night she dances garba for two hours. She becomes shaky, sweaty, and confused and has to be helped to a chair. Talking with her prescriber before the festival, choosing a partial fast with paneer, yogurt, and nuts, eating before garba, and carrying glucose tablets would very likely have let her enjoy all nine nights.

The short version

Before Navratri, talk to your clinician, especially if you take insulin or a sulfonylurea, and never change those medicines on your own. Choose a fast that fits your health, build vrat meals around paneer, yogurt, milk, and nuts, and go easy on fried foods. Eat before garba, carry water and glucose tablets, and break the fast if you feel unwell. Ask about holding dose increases, never skip doses as part of the fast, and keep a simple log.

If you want a provider who will help you plan treatment around your faith and festivals, TrimRx includes ongoing provider access alongside treatment.

This information is for educational purposes and is not medical advice.
