A stomach bug or food poisoning can also strike at any time. If you take the Wegovy pill once a day, vomiting raises an obvious question: did that dose count, and should you take another?

If you take the Wegovy pill or another GLP-1 in tablet form, here is what to know. The most important rule: do not take a second pill without checking with your pharmacist or provider first.

When to get help right away

Call your provider the same day, or seek urgent care, if you have any of these:

Vomiting or diarrhea that stops you from keeping fluids down

Very little urine, very dark urine, or dizziness that does not ease

Severe stomach pain that will not go away, with or without vomiting

Signs of a serious allergic reaction, such as swelling of the face, lips, or throat, or trouble breathing (call 911)

Why dehydration is the real worry

The prescribing information for the Wegovy pill notes postmarketing reports of acute kidney injury in people treated with semaglutide, and advises monitoring kidney function in people reporting side effects that could lead to volume depletion, especially when starting or increasing the dose. Repeated vomiting is exactly that situation. Sip fluids steadily, such as water, broth, or an electrolyte drink, in small amounts every few minutes, and call your provider if you cannot keep them down.

Should you take another pill?

The label gives a clear rule for missed doses: skip the missed dose and take the next dose the following day. It does not give a separate rule for vomiting after a dose. Because of that, do not take another tablet on your own. Call your pharmacist or provider and tell them when you took the pill and when you vomited. Unless your pharmacist or provider tells you otherwise, wait and take your next pill the following morning as usual. Never take two pills to make up for one.

What the side effects look like

The prescribing information for the Wegovy pill says the types and frequency of common side effects were similar to those of the Wegovy injection. In the injection trials, nausea was reported by 44% of people on the medication against 16% on placebo, and vomiting by 24% against 6%. Most digestive side effects are strongest early in treatment and after each dose step. If you are comparing medications, our guide on tirzepatide and semaglutide side effects covers how they differ.

If it happens after a dose step

The Wegovy pill starts at a low dose that steps up about every 30 days over the first months. If vomiting began after a step up, tell your provider. They may want to change the pace. Our guide on semaglutide dose escalation covers how dose steps usually work.

Eating again

Once you can keep fluids down, start with small amounts of plain food, such as crackers, toast, rice, bananas, or broth. Add protein gradually as you feel better. Our guide on what to eat to minimize nausea has more ideas.

Making the next morning go better

Take the pill exactly as the label says: on an empty stomach, with up to 4 ounces of plain water, swallowed whole, then wait at least 30 minutes before food, drinks, or other medicines. When the wait is over, eat a small, gentle breakfast rather than a big, rich one. Avoid lying down right after eating.

If you have diabetes

The prescribing information for the Wegovy pill says it “lowers blood glucose and can cause hypoglycemia,” and that in diabetes trials, the risk was increased when semaglutide was used with insulin or an insulin secretagogue, such as a sulfonylurea. If you are vomiting and cannot eat, ask your diabetes care team what to do about those medicines that day. Never change them on your own.

If vomiting keeps happening

Occasional nausea is common. Vomiting that happens often is not something to push through. Tell your provider rather than quietly stopping the medication. Our guide on taking a break and restarting covers why stopping and restarting is more involved than it sounds.

Track it

Write down when you took your pill, when you vomited, what you ate, and how you felt, and share it with your provider. Watch for the signs of under-eating too: persistent fatigue, feeling cold, hair shedding, dizziness when you stand, losing strength, and losing weight faster than you and your provider intended. Our guide on tracking your progress covers ways to record more than your weight.

Consider this scenario: a woman on the Wegovy pill throws up an hour after her morning dose, the week after a dose step. Worried the dose did not count, she takes a second pill at lunch and vomits again that afternoon. By evening she is dizzy and barely urinating. Calling her pharmacist before taking another tablet, sipping fluids in small amounts, waiting until the next morning for her usual dose, and calling her provider when she could not keep fluids down would very likely have kept her out of trouble.

The short version

If you throw up after taking the Wegovy pill, do not take another one without checking with your pharmacist or provider, and never double up. Sip fluids in small amounts, and get help the same day if you cannot keep fluids down, have very little urine, or have severe stomach pain. Restart with gentle foods, take your next pill as directed, and tell your provider if vomiting began after a dose step or keeps happening.

If you want a provider you can reach when side effects come up, TrimRx includes ongoing provider access alongside treatment.

This information is for educational purposes and is not medical advice.
