Do You Need to Pinch the Skin When Injecting Semaglutide?
It depends on your needle length and your body, and for most adults using a short pen needle in the abdomen, the answer is no. The skin fold exists to solve one problem: making sure a needle does not reach muscle. When the needle is short relative to your subcutaneous tissue, that problem does not arise, and pinching adds a step without adding protection.
The situations where a fold genuinely helps are the opposite ones: a longer needle, a leaner build, or a site with less tissue between skin and muscle, such as the thigh or the back of the upper arm.
Your instructions for use are the authority for your specific device. If they say to pinch, pinch.
Why skin thickness settles this
The reasoning rests on two measurements: how thick your skin is, and how much subcutaneous fat sits between it and the muscle beneath.
A study using ultrasound to measure both in 388 adults across a wide range of body sizes found something useful and slightly counterintuitive. Skin thickness itself varies remarkably little between people, whether they are lean or carrying substantial weight. Subcutaneous fat thickness varies far more, and it differs by site.
The practical consequence is that a short needle reliably clears the skin and lands in fat in almost everyone, which is why modern pen needles trend short. Where you have less fat, a longer needle can travel past it into muscle, and that is when the fold earns its place.
When a skin fold helps
A longer needle. If your device or your prescribed syringe uses a longer needle, a fold increases the distance to muscle.
A lean build. Less subcutaneous tissue means less margin.
Thigh or upper arm sites. Both generally have less subcutaneous depth than the abdomen.
When you tend to press hard. Pressing the device firmly into the skin compresses tissue and effectively shortens the distance to muscle. A fold offsets that, though the better fix is to press less.
When your instructions say so. Different devices come with different guidance, and yours governs.
When it does not help, and can hurt
Short needle in the abdomen for most adults. The fold is unnecessary and adds a way to get it wrong.
When you grab too much. This is the most common error. Squeezing a large handful of tissue with your whole hand often lifts muscle along with fat, which defeats the entire purpose and can make an intramuscular injection more likely rather than less.
When you squeeze hard. A tight pinch is more painful and compresses the tissue you are trying to inject into.
When you release it too early. Letting go while the needle is still in changes the tissue geometry mid-injection and can push medication back toward the surface.
Consider this scenario: a patient reads that pinching is safer, grabs a firm handful of their abdomen with their whole hand, injects, and lets go immediately after the counter reaches zero. They have introduced three separate errors while trying to add a safety step. Not pinching at all would have been better than that.
How to do it correctly, if you do
- Use your thumb and two fingers, not your whole hand
- Lift gently, enough to raise skin and fat away from the muscle beneath
- Do not squeeze hard, and do not blanch the skin
- Insert the needle at ninety degrees into the raised fold
- Deliver the dose at a controlled pace
- Keep holding the fold and keep the needle in place for a slow count of six after the counter reaches zero
- Withdraw the needle, then release the fold
Step seven is the order people reverse. Needle out first, fold released second.
What matters more than the pinch
If you are optimizing your technique, these three do more work:

A fresh needle every time. Reuse blunts the tip, which increases pain, bruising, and tissue trauma.
The hold after the dose. Keeping the needle in place for several seconds after delivery prevents most avoidable leakage, and it makes a bigger practical difference than whether you pinched.
Ninety degrees or forty-five?
This question travels with the pinch question and has largely been settled the same way. With short needles, a straight ninety-degree insertion is the standard approach and is what current guidance and most device instructions describe. The forty-five degree angle belongs to an era of longer needles, where angling was a way to avoid going too deep.
If you are using a longer needle, particularly with a syringe drawn from a vial, your instructions may specify an angle. Follow them rather than a general rule.
If you use a vial and syringe
Needle length and gauge for syringes are part of your prescription and were chosen deliberately. Do not switch lengths on your own, and do not assume advice written for a pen needle applies to your syringe.

When to ask your provider
Ask if you are very lean and unsure whether your needle length gives you enough margin, if you have been injecting into the thigh or arm and getting more soreness than expected, if your instructions for use are unclear or you no longer have them, or if you have switched devices and are not sure whether the old technique still applies.
