Doing a Polar Plunge on Wegovy

Running into freezing water on New Year’s Day has become a tradition in towns and beaches across the country, often for charity and almost always with a crowd. Cold plunges and winter swims are popular year-round too. If you are on Wegovy or compounded semaglutide, the medication does not rule this out. But cold water puts real stress on the body, and a few things about treatment are worth thinking through first.

What cold water does in the first minute

Sudden immersion in cold water triggers a gasp and rapid, uncontrolled breathing, and pushes heart rate and blood pressure up sharply. This cold shock response is strongest in the first minute or so. It is the main reason cold water is dangerous even for strong swimmers, and why organized plunges keep people close to shore with safety crews in the water.

For most healthy people, a short, supervised dip is over before it becomes a problem. For some people, that sudden strain on the heart is a real risk.

Check with your doctor first if your heart is involved

Wegovy carries an indication to reduce the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight. That indication rests on the SELECT trial, which enrolled 17,604 people aged 45 or older who had established cardiovascular disease and a BMI of 27 or above, and who did not have diabetes.

If you take Wegovy partly for heart protection, or you have heart disease, high blood pressure, or a heart rhythm problem, ask your cardiologist before you plunge. The medication’s benefit sits alongside your cardiac care, not in place of it. The label also includes a warning titled Heart Rate Increase. Mention any racing or pounding heartbeat at rest to your provider, plunge or not.

Why you may feel the cold more

Many people on these medications say they feel colder than they used to. Some of that may simply reflect eating less. But feeling cold all the time is also one of the signs of under-eating, along with persistent fatigue, hair shedding, dizziness when you stand, losing strength, and losing weight faster than you and your provider intended. If that sounds familiar, tell your provider before you add a winter plunge to the mix.

Eat and drink beforehand

Do not plunge on an empty stomach. Have a proper breakfast with some protein a couple of hours before, and a warm drink while you wait. Many plunges involve long lines in the cold, which drains energy before you even reach the water. Our guide on what to eat after an injection has ideas for foods that tend to sit well.

Plan your dose around the day

Side effects tend to be strongest in the first weeks and after each dose increase, and many people notice them most in the day or two after an injection. In the trials, dizziness was reported by 8% of adults on Wegovy compared with 4% on placebo.

Do not step up your dose right before the plunge. Ask your provider to hold it until afterward. Our guide on semaglutide dose escalation covers how the steps usually work. If you only started recently, our guide on the semaglutide starting dose covers what the early weeks feel like, and that may be reason enough to cheer from the beach this year.

If you have switched from tirzepatide, our guide comparing tirzepatide and semaglutide side effects covers how the two compare.

Skip the drinks until you are warm

Alcohol widens blood vessels near the skin, which makes you feel warmer while you actually lose heat faster. On a smaller appetite, it also tends to hit harder. If the plunge follows a late New Year’s Eve, you may be short on sleep and fluids before you even start. Save any celebrating for after you are dry and warm, and have it with food.

In the water

Go in with a buddy and stay where the safety crew can see you.

Walk in rather than diving, and keep your head above water.

Keep it short. A quick dunk counts.

Get out immediately if you feel lightheaded, confused, or unable to control your breathing.

Warming up afterward

Have dry clothes, a hat, a towel, and warm shoes ready so you can change immediately. Get out of wet clothes first, then layer up and have something warm to drink and eat. Shivering that will not stop, confusion, or slurred speech need medical help.

Consider this scenario: someone on Wegovy steps up their dose on December 30, stays out late on New Year’s Eve, and skips breakfast before a 10 a.m. plunge. They wait an hour in the wind, dive in, stay in to film a video, and then stand around in wet clothes chatting. They feel faint and shaky and need help from the medical tent. Holding the dose increase, eating breakfast, walking in briefly with a buddy, and changing into dry clothes straight away would very likely have kept it a fun morning.

The short version

A short, supervised polar plunge can be fine on Wegovy, but cold shock is real. Ask your cardiologist first if you have heart disease or take Wegovy for heart protection. Eat beforehand, do not step up your dose right before the day, skip alcohol until you are warm, go in briefly with a buddy, and have dry clothes ready. If you feel cold all the time, raise it with your provider.
