Ramadan on the Wegovy Pill
During Ramadan, many Muslims fast from dawn until sunset, eating suhoor before dawn and breaking the fast at iftar. For people on a weekly injection, the main question is how the medication affects fasting. For people on the Wegovy pill, which is taken every day on an empty stomach, there is a second question: when to take it.
If you take the Wegovy pill or another GLP-1 in tablet form, here is what to think about and what to discuss with your clinician. Islam exempts people whose health would be harmed by fasting, and the decision about whether and how to fast belongs to you, your clinician, and your imam.
What the label says
The prescribing information for the Wegovy pill says to take one tablet once daily in the morning on an empty stomach, with water, up to 4 ounces, and not with any other liquid. Then wait at least 30 minutes before eating food, drinking beverages, or taking other oral medicines.
During Ramadan, that 30-minute wait has to fit inside the short window before dawn, so the pill and suhoor have to be planned together.
Talk to your clinician before Ramadan
Ask your prescriber well before the month begins:
- When should I take the pill during Ramadan?
- If I take it before suhoor, how early do I need to wake up to fit the 30-minute wait before eating?
- Does anything about my other medicines need to change?
- How should I check my blood sugar if I have diabetes?
Do not change when or how you take the pill without that conversation.
A pre-dawn routine to discuss
One approach people discuss with their clinician is waking a little earlier, taking the pill with a small amount of water, waiting at least 30 minutes, and then eating suhoor and drinking well before dawn. That can mean an alarm 45 minutes or more before the fast begins. Your clinician can tell you whether that fits your situation.
If you have diabetes
The prescribing information for the Wegovy pill says it “lowers blood glucose and can cause hypoglycemia,” and that in diabetes trials, the risk was increased when semaglutide was used with insulin or an insulin secretagogue, such as a sulfonylurea. Long fasting days make that risk higher. If you take either, talk to your prescriber before Ramadan, and never change those medicines on your own. Break the fast and treat a low right away if you feel shaky, sweaty, confused, or faint.
Suhoor on a small appetite
Once the 30 minutes have passed, make suhoor count. Eat protein first, such as eggs, yogurt, cheese, beans, or lentils, with some slow-burning carbohydrate like oats or whole-grain bread. Drink water steadily until dawn. Our guide on what to eat to minimize nausea has ideas for foods that tend to sit well.
Iftar without overdoing it
After a long fast, it is tempting to eat quickly. On the Wegovy pill, you will fill up fast. Break the fast with water and dates, pause for prayer, then eat a small plate with protein. Fried foods and rich sweets are the most likely to cause nausea, so keep them small.
Hydration through the night
Long days without water and the digestive side effects of these medications both add up. Drink water steadily between iftar and suhoor. If you are vomiting or have diarrhea, talk to your clinician about whether to keep fasting.
Dose steps during Ramadan
The Wegovy pill starts at a low dose that steps up about every 30 days over the first months, and side effects tend to be strongest after each step up. If a step up would fall during Ramadan, ask your provider whether to wait. Our guide on semaglutide dose escalation covers how dose steps usually work. If you are comparing medications, our guide on tirzepatide and semaglutide side effects covers how they differ.
If you miss a dose
If you oversleep and there is no time for the 30-minute wait before dawn, do not take the pill with suhoor. The label says to skip a missed dose and take the next dose the following day.
Protein through the month
A month of short eating windows and a small appetite can leave you short on protein. Some of the weight lost on these medications is muscle. Our guide on getting the most out of your GLP-1 treatment covers why protein matters. Keep a simple log of what you eat and how you feel. Our guide on tracking your progress covers ways to do that.
Watch for the signs of under-eating: persistent fatigue, feeling cold, hair shedding, dizziness when you stand, losing strength, and losing weight faster than you and your provider intended.
Consider this scenario: a man on the Wegovy pill who also takes a sulfonylurea decides on his own to take his pill with his suhoor meal to save time. He eats very little at iftar because he fills up fast, and in the second week he feels shaky and confused in the late afternoon. Talking with his clinician before Ramadan, waking earlier to take the pill and wait 30 minutes before suhoor, eating protein at both meals, and breaking the fast when he felt unwell would very likely have kept him safe.
The short version
Before Ramadan, ask your clinician how to time the Wegovy pill, since it must be taken on an empty stomach with water and followed by a 30-minute wait. A routine to discuss is waking earlier, taking the pill, waiting, then eating suhoor. If you take insulin or a sulfonylurea, plan the fast with your prescriber and break it if you feel unwell. Eat protein at suhoor and iftar, drink water through the night, and skip a missed dose rather than taking it with food.
