Why Are My Lips Always Dry on Tirzepatide?

Almost always because you are drinking less than you used to, and you have probably not noticed.

Appetite suppression reduces how much you eat and drink, a meaningful share of daily fluid normally comes from food, and thirst becomes a much weaker prompt. The result is a slow slide into mild dehydration, and lips are frequently where it shows first because they have no oil glands and lose moisture faster than the rest of your skin.

If your lips are persistently dry, treat it as a signal about your fluid intake rather than a cosmetic problem to solve with balm.

Why lips specifically

Lip skin is thin, has no sebaceous glands, and is constantly exposed. It has almost no capacity to hold moisture on its own, which is why it shows a fluid deficit before your arms or face do.

Add three things common on this medication:

Mouth breathing, particularly at night, which dries them further.

Lip licking, which feels like it helps and makes it worse, since saliva evaporates and takes moisture with it.

Reduced saliva generally, which is common alongside reduced fluid intake.

Consider this scenario: a patient buys three different lip balms over two months and applies them constantly, with no lasting improvement. She is drinking roughly two glasses of water a day because she is never thirsty. The balm was treating the surface of a problem that was systemic, and increasing her fluid on a schedule resolved it within a fortnight.

The fix that actually works

Drink on a schedule, not on thirst. This is the whole answer for most people. Decide on an amount, split it across the day, and use a bottle you refill a known number of times. Waiting for thirst is what failed you.

Add electrolytes if you have had any vomiting or diarrhea, or if plain water alone is not resolving it. Our guide on electrolytes on semaglutide covers why this matters more on this medication than most people expect.

Stop licking them. The single most effective behavioural change, and the hardest.

Use an occlusive balm, meaning something that seals moisture in rather than a flavoured one that encourages licking. Plain petroleum jelly or a simple ointment works better than most products marketed for it.

Apply before bed, when lips dry most.

Use a humidifier if you wake with dry lips and mouth.

Breathe through your nose where you can.

Sun protection. Lips burn easily and sun damage worsens dryness. A balm with SPF is worth using outdoors.

The deficiencies worth checking

If dryness persists despite genuinely improved hydration, and particularly if it comes with cracks at the corners of your mouth, that is worth investigating rather than continuing to apply balm.

Iron deficiency causes cracking at the corners of the mouth and is plausible when intake has been reduced over months. Our guide on iron deficiency on semaglutide covers why it happens on this medication.

B vitamin deficiencies, particularly B2 and B12, produce similar changes.

Zinc, which affects skin healing generally. Our guide on zinc supplementation on semaglutide covers what to know.

Ask for testing rather than starting supplements speculatively, since iron in particular should not be taken without a documented deficiency. Our guide on what lab tests to expect while on GLP-1 medications covers what is typically monitored.

Cracks at the corners specifically

Worth separating, because they have a distinct set of causes.

Cracking, redness, or soreness at the corners of the mouth is not simply dry lips. It can reflect nutritional deficiency, a fungal or bacterial infection, or saliva pooling at the corners.

It is worth showing a provider or dentist rather than treating with balm, because if it is infective it needs a different treatment entirely and balm will not touch it.

The cluster worth noticing

Dry lips rarely arrive alone on this medication.

If you also have dry mouth, headaches, constipation, fatigue, or dizziness on standing, those are not five separate problems. They are the recognizable pattern of inadequate fluid, and correcting hydration usually improves several of them together.

Persistently dry lips alongside feeling cold all the time, hair shedding, and losing strength is a different and more concerning cluster, pointing at inadequate intake generally rather than just fluid. That is worth a call. Our guide on eating enough protein on tirzepatide covers what adequate looks like.

Tell your dentist

Worth mentioning at your next appointment, since dry lips usually travel with dry mouth, and dry mouth raises the risk of cavities and gum disease over time.

Your dentist will want to know you are on this medication, and it changes what they monitor.

When to call your provider

Contact your provider if:

  • Dryness persists despite genuinely increased fluid intake
  • You have cracks or sores at the corners of your mouth
  • Lips are cracked, bleeding, or painful
  • You have signs of dehydration including dark urine, reduced urination, or dizziness on standing
  • Dryness comes with fatigue, feeling cold, or hair shedding
  • You have mouth ulcers or a burning sensation

The short version

Lips have no oil glands, so they show a fluid deficit first, and on this medication you are almost certainly drinking less than you used to because thirst stopped prompting you. Drink on a schedule, stop licking them, and use a plain occlusive ointment rather than a flavoured balm. If it persists despite proper hydration, or you have cracks at the corners, that is worth testing rather than treating.
